The forms are online and take five minutes. We send the link when you book.
A driver's license or passport, for your record.

Or a photo of both sides. We check your cover before you sit down.
A list, or the boxes. Some change how we numb a tooth.
Most insurance plans pay all of it as preventive care. Without insurance, the $29 membership covers it.
